In reply We were pleased to see the testing of IPT by Stuart and O'Hara for the depression of women in the postpartum period. The modifications they have suggested dealing with marital disputes and the role transition are part of the IPT framework. We believe that psychotherapy should be available to women with depression who may not be able to take medication. Women with postpartum depression who wish to nurse may be good candidates, since drugs are secreted into the breast milk and the effect on the offspring could be problematic.We have two comments. We would urge the investigators to undertake the controlled and randomized treatment trial comparing IPT with standard pharmacologic treatment for postpartum depression, and, if their sample size allows, compare it with another type of psychotherapy.Stuart and O'Hara note that psychotherapy is emphasized for conditions for which there are as yet no effective treatments or for conditions
