In reply We would like to comment on the report by Pollmächer et al as well as present some data on patients at our center. The results reported by Pollmächer et al reveal an interesting trend. The levels of SIL-2R began to increase immediately, peaking at about 2 weeks. This is followed by a decrease that is noted at the end of the 6 weeks of observation. It is thus conceivable that the levels of SIL-2R may in fact return to baseline with continuing clozapine treatment; however, they did not measure SIL-2R levels beyond 6 weeks of treatment.In view of these interesting results, we examined the relationship between SIL-2R levels in groups of patients who were being treated with different antipsychotic drugs to see whether patients treated on a long-term basis with clozapine had higher SIL-2R levels than did patients receiving long-term treatment with other antipsychotics.
